Meet Donald. Donald is a 50 year old dually diagnosed consumer, working through disabilities of Schizophrenia and Moderate Mental Retardation. Donald grew up in state hospital for the developmentally disabled, but transitioned into community living. He has struggled adapting and has been in and out of psychiatric hospitals, but needs tighter integration of care that a Health Home can provide.
Meet Christine. Christine is a 38 year old who has struggled with drug and alcohol addictions. This has resulted in Christine being in and out of prison, transitioning between jail cells, half way houses and hospitals.
Meet Robert. Robert is a 68 year old, who has struggled with Bipolar Depression his entire life. He has tried to transition into community group homes, but many of these attempts failed in psychiatric hospitalization and court involvement. Robert was recently diagnosed with Alzheimer's and will be moving into a long term care facility.
Meet Rosa. Rosa just turned 14 and has been placed in a school for autistic children, after failed attempts with foster families, local school programs and developmental centers. She is now getting much needed help as she prepares to enter supportive living programs once phasing out of school.
Meet Amy. Amy moved in and out of foster care most of her life, struggling to find a good family that could see past her anxiety disorder and substance use problems. Amy has battled her addictions with stints in and out of rehab, and is getting much needed therapy to combat her family loss issues.
