All too often, our psychiatric inpatient facilities function in a disconnected fashion from the community based system. However, psychiatric hospitalization is not a destination point, but a stop along the road to recovery and an integral part of the behavioral healthcare continuum. In this healthcare environment, new clinical coordinated care management practices are emerging, along with financial drivers to more fully integrate the hospital with the community.
Discharge Planning Best Practices
The new standard of practice is to discharge the patient in a timely fashion to the community and for the community to have in place the necessary resources to sustain a successful community placement for the patient. This 30-minute educational event features renowned industry experts Dr. Jess Jamieson and Christine Holmes of CoCENTRIX. The webinar’s discussion answers the following questions:
Why the interest in the discharge and transition process from hospital to community? What are the clinical and economic drivers for this process?
What are the best clinical practices for this process? What are the “core” features of a process for the timely discharge of a patient and the care coordination activities
necessary for the patient to remain in the community?
At the end of the day, how realistic is it that we can get this job done with the always limited resources available in the behavioral healthcare arena?
What technologies can help facilitate the support, accountability, and coordinated care needed for a positive transition?
